Request a Baptismal Certificate
- For all Baptized at St. Thomas Becket, we maintain a record of all Sacraments celebrated during a person's lifetime.
  
  If you need a Baptismal Certificate to celebrate another Sacrament and were Baptized at St. Thomas Becket, we can help.
  
  To request a Baptismal Certificate, you may contact our office in the following ways:

  via the adjacent request form.
  
  For those not Baptized at St. Thomas Becket, we cannot provide Baptismal Certificates, even if you celebrated other Sacraments with us. Contact the church where the person received the Sacrament of Baptism for assistance.
